I am posting a warning to ALL Intel employees or employees required to go through an out-sourced agency that dictates approval for FMLOA (Family Medical Leave of Abscence). In order for you to get your FMLOA approved, you will need specific documentation that "most" urgent care facilities/physicians/hospitals will sign without hesitation. However, given the fact that Maricopa Urgent Care is a "PRIVATELY OWNED" facility, they will NOT sign any documentation of this nature. This is something I feel could be posted for customers to make their own determination as to whether this facility fits their need BEFORE they take ANY time off for FMLOA. At which time, it becomes retroactive to get your paperwork signed by the doctor for approval by the agency governing your FMLOA. This serves as a warning for Intel employees who may need FMLOA. Please go to your primary care physician, the ER, or an urgent care facility that will sign your FMLOA paperwork, because Maricopa Urgent Care wil NOT.

Maricopa Urgent Care needs to be shut down. There have been THREE separate occassions that I’ve been in there and had misdiagnoses’. Yesterday about caused a tragedy. 90% of the time you go to MUC, they don’t have an MD in the building, they are generally PA’s or NP’s. You wait forever, they don’t keep you in order of "arrival" they have put up a fuss about my insurance EVERY TIME even though I’ve been there numerous times and they’ve gotten paid every single time.
Yesterday, my "NP", Scott Van Skike, after reading my "symptoms" marked on that tiny piece of paper they have you fill out at the beginning came into the room, sat on his "royal throne" (aka that spinning stool all kids love to play with while waiting for hours on end) and looked at me and said, "I think we’re looking at an Upper Respitory Infection today." When my symptoms listed were, "fever, diarrea for 24 hours straight, no eating/drinking, no urination for at least 12 hours. I asked him if he might be in the wrong patient room cause I’m not sure where he got that idea. He too hadn’t even TOUCHED my child to "evaluate" him, he had only gone on my symptom list. After "touching him/evaluating him" he went on to tell me my son had CONSTIPATION!!!!! I kid you not! Did the man have a brain? I’m not sure, but apparently not! After leaving there, I immediately went and had him reevaluated, long story short for you all, he was put on IV for severe dehydration. Had I just taken my son home and not gone on, we could’ve had a very serious tragedy!
In short, DON’T EVER GO TO THE MARICOPA URGENT CARE! The other two misdiagnoses’ were diagnosed as "viral infection" when they were clearly PNEMONIA. Please take this seriously! One of these days, someone walking into that Urgent Care will DIE because of this facilities lack of REAL MEDICAL ATTENTION!
